The FDA has launched the Rare Disease Framework Initiative, the agency announced on October 14. The initiative aims to develop a fit-for-purpose framework for developing drugs and biologics for rare diseases.
A new FDA white paper outlines the framework. It is intended to account for the nuances of each disease, patient needs, product type, and the evidence that can realistically be generated in small patient populations. The agency said only about 5% of rare conditions have an FDA-approved treatment. It also said it agrees with patients, drug developers, and providers that more must be done to speed the development of safe and effective treatments.
In developing the framework, the FDA said it is committed to providing greater clarity, consistency, and predictability in regulatory decision-making. Its other commitments include bringing in the patient’s voice early and at defined milestones, making more use of external expertise, and building shared resources and training. The agency also listed applying risk-based approaches, particularly to chemistry, manufacturing, and controls review and preapproval inspections, and championing novel methods and innovative statistical approaches.
The FDA’s Rare Disease Innovation Hub leads the initiative, working with the Office of the Commissioner, the Center for Biologics Evaluation and Research, and the Center for Drug Evaluation and Research. “This new framework will enhance consistency and predictability for both sponsors and for patients,” said Amy Comstock Rick, JD, director of the Hub.
The FDA will host a public meeting on October 28 at its White Oak campus to gather feedback from patients, disease advocates, industry, and others. The Reagan-Udall Foundation for the FDA will facilitate the meeting. The agency said more opportunities for input will follow, including sector-specific roundtables, requests for information, and internal feedback sessions. According to the FDA, that feedback will shape a framework that might include new or revised guidance, changes to regulations, staff training, and organizational changes.
