Sodium-glucose cotransporter-2 (SGLT2) inhibitors reduced the composite of first hospitalization for heart failure (HHF) or cardiovascular (CV) death by 23% compared with placebo in adults with type 2 diabetes, according to a meta-analysis of randomized controlled trials published in
Diabetes, Obesity and Metabolism. The investigators noted that although SGLT2 inhibitors have demonstrated cardiorenal benefits in landmark trials, cross-trial inconsistencies in CV mortality, renal endpoints, and drug-specific effects have left class-level estimates incompletely synthesized. They searched PubMed, Web of Science, Scopus, and CENTRAL from inception through May 15, 2026, for large placebo-controlled trials (≥1000 participants) and prespecified subgroup analyses in adults with type 2 diabetes, and pooled hazard ratios using random-effects models.
The analysis included 14 studies with 76,969 patients (41,761 receiving SGLT2 inhibitors and 35,208 receiving placebo). The primary composite outcome was reduced with SGLT2 inhibitors (hazard ratio [HR], 0.77; 95% CI, 0.73-0.81), with no heterogeneity across studies (I²=0%) and no effect modification by baseline heart failure or established ASCVD.
SGLT2 inhibitors also reduced 3-point major adverse cardiovascular events by 14% (HR, 0.86; 95% CI, 0.81-0.92), driven by significant reductions in CV death (HR, 0.86; 95% CI, 0.79-0.93) and myocardial infarction (HR, 0.88; 95% CI, 0.81-0.96), with no significant effect on stroke (HR, 0.93; 95% CI, 0.81-1.08). First HHF was reduced by 29% (HR, 0.71; 95% CI, 0.66-0.77) and composite kidney outcomes by 31% (HR, 0.69; 95% CI, 0.60-0.79).
The authors noted that moderate heterogeneity in kidney outcomes should be interpreted in the context of trial-level clinical differences and nonstandardized kidney endpoint definitions. They concluded that the findings support guideline-concordant initiation of evidence-based SGLT2 inhibitors as foundational therapy in type 2 diabetes.
