A structured digital program delivering glucagon-like peptide-1 receptor agonist (GLP-1RA) therapy, almost entirely tirzepatide, to more than 8,000 women in the UK was associated with a mean weight reduction of 15.5% at 6 months, according to an observational study published in
Nature Medicine. The investigators noted that large cardiometabolic outcomes trials in obesity have enrolled predominantly male populations and that real-world evidence for digitally delivered care remains limited. The program, jointly operated by SheMed and eMed Healthcare UK, which employ two of the authors, combined remote eligibility assessment, at-home blood testing, weekly video check-ins, and monthly clinician review with individualized dose titration, without routine in-person visits.
The analysis included 8,127 adult women with a mean baseline body mass index of 38.2kg/m². For the primary endpoint, mean weight decreased by 16.1kg among the 7,297 participants with 6-month weight measurements. Of these, 83.1% lost at least 10% of baseline weight, and 22.4% lost at least 20%. Mean tirzepatide dose was 9.5mg weekly at 6 months.
Among participants with paired blood tests, mean glycated hemoglobin decreased by 3.3mmol/mol, from approximately 5.5% to 5.2%, normalizing in 84.5% of those with baseline diabetes-range values and 96.3% of those with prediabetes. Total cholesterol, low-density lipoprotein cholesterol, and triglycerides decreased by 0.77, 0.61, and 0.47mmol/L, respectively, while high-density lipoprotein cholesterol declined modestly. Retention was 91.4% at 6 months. Overall, 62.7% of participants reported at least one side effect, with the most severe rated as severe in 7.7% of them; no serious treatment-related adverse events were identified.
The authors cited the observational design, absence of a comparator group, self-pay population, and 6-month follow-up as limitations and said causal attribution to GLP-1RA therapy cannot be definitively established. They also noted the cohort was predominantly white and generally healthy, cautioning against generalizing the findings to people with greater obesity-related complications.
