The American Academy of Pediatrics (AAP) recommends 2026–2027 COVID-19 vaccine for all infants and children 6 through 23 months of age who do not have contraindications, according to an updated policy statement published in Pediatrics. The AAP also recommends a single dose for children and adolescents 2 through 18 years of age in specified risk groups, regardless of prior vaccination status.
The statement, developed by the AAP Committee on Infectious Diseases, updates the organization’s recommendations on COVID-19 vaccines for preventing severe COVID-19 in children. A companion technical report reviews the supporting evidence. The AAP noted that the recommendations may change in future seasons or as additional variants emerge.
The AAP describes children 6 through 23 months of age as being at high risk for severe COVID-19. Under the statement, previously unvaccinated children in this age group should receive an initial series. Those who started but did not complete the series should complete it, and those who completed it should receive a single dose at least 8 weeks after the last dose. Children 6 months through 18 years of age who are moderately or severely immunocompromised require 2 or more doses, depending on prior vaccination status.
For ages 2 through 18 years, the risk groups include children at high risk of severe COVID-19, residents of long-term care facilities or other congregate settings, those never vaccinated against COVID-19, and those whose household contacts are at high risk. Children outside these groups whose parent or guardian wants them protected should be offered a single dose. The AAP said any age- and health status–appropriate vaccine approved by the FDA through a biologics license application can be used, and the most updated available version should be used.
The statement advises vaccination as soon as the updated vaccine is available, rather than waiting for a particular point in the season. It notes that COVID-19 vaccine may be given at the same visit as seasonal influenza vaccine. Children not vaccinated in the fall can still be vaccinated at any time during the season. The AAP said the guidance does not indicate an exclusive course of treatment or serve as a standard of medical care.
