Adding a proprotein convertase subtilisin/kexin type 9 (PCSK9) inhibitor to statin therapy improved functional independence and reduced recurrent ischemic stroke compared with statin monotherapy in patients with acute ischemic stroke (AIS), according to a systematic review and meta-analysis published in the
Journal of Neurology. The authors described the evidence as preliminary.
The investigators noted that PCSK9 inhibitors have shown benefits across many cardiovascular patient groups but that evidence specific to AIS had not been quantitatively pooled. Their analysis compared the efficacy and safety of PCSK9 inhibitor plus statin therapy with statin monotherapy.
Researchers from the First Affiliated Hospital of Guangxi Medical University in Nanning, China, searched seven databases from inception through May 31, 2026, for randomized controlled trials (RCTs). Compared with statins alone, combination therapy was associated with higher odds of functional independence (modified Rankin Scale score 0-2; odds ratio [OR], 2.17; 95% CI, 1.29-3.64) and lower odds of recurrent ischemic stroke (OR, 0.33; 95% CI, 0.16-0.69).
The authors reported a substantial LDL-C reduction in the combination group (mean difference, 0.66 mmol/L; 95% CI, 0.45-0.87), with high heterogeneity partly explained by statin type (atorvastatin vs rosuvastatin). National Institutes of Health Stroke Scale (NIHSS) scores did not differ significantly between groups. Abnormal liver function, major adverse cardiovascular events, and total adverse events were comparable between groups.
Using the
GRADE approach, the authors rated evidence certainty as moderate for functional independence; low for recurrent stroke, LDL-C, and safety outcomes; and very low for NIHSS. They concluded that adding a PCSK9 inhibitor may improve functional recovery and reduce recurrence in statin-treated patients with AIS, with acceptable short-term safety, but said the findings require confirmation in larger, adequately powered RCTs.
